Bariatric surgery

Many people today are under the impression that weight loss surgery is a quick fix for losing weight. Unfortunately for those people who are Bariatric (morbidly obese), this type of weight loss surgery can carry its own risks
There are some in the medical profession who would consider Bariatric surgery to be a life saving operation and indeed in many cases they would be right. Unfortunately due to the very nature of their obesity problem, the surgery itself carries its own risks with some Bariatric patients ultimately dying on the operating table.
Bariatric surgery does not come under the same umbrella as cosmetic surgery as the whole point of this surgery is not to particularly enhance a person’s looks, but to ultimately improve their lifespan.
Very careful considerations need to be given to anyone who is considering Bariatric surgery. Surgeons often have a very difficult choice to make as the complications which arise from not only the surgery but also the anaesthesia can put their patient at risk. Usually the surgeon has to weigh up the decisions he makes by deciding whether the patient is more likely to die with or without the surgery.
Specific guidelines have to be followed when deciding whether someone is a good candidate for Bariatric surgery and these include considerations such as whether they have
* medical problems associated with their obesity,
* a body mass index of over 40
* They can prove they have failed on medically supervised conservative diets
* They are at risk of developing further severe health problems
Anyone who is considering Bariatric surgery needs to ask themselves some serious questions, and if possible attend some type of counselling course to ensure they don’t have any unrealistic expectations of what they will be like after their surgery.
As stated it should be remembered that Bariatric surgery is not cosmetic surgery, and people should realise that there is still a long way to go after they have had their procedure. Bariatric surgery does not make you a nicer person and it doesn’t make people like you more. Many people cling onto ideas such as this and are devastated when they realise their surgery is not going to ultimately change the person that they are.
Many Bariatric people are carrying emotional baggage with them and their obesity is part of this baggage. Indeed for some though they may not realise it, it is their excuse to hide away from the world and not have to face up to specific problem areas of their life. Having the surgery can sometimes be a rude awakening and often backfires on them with the ultimate consequences of them deliberately sabotaging their weight loss.
For others Bariatric surgery is the option that finally helps them to conquer their obesity problem and is often the jumpstart they need to enhance their lives and their health. This includes a longer lifespan, a better self image and improved self esteem.

Liposuction

If you are considering going for liposuction surgery, then you need to know the facts first.
Liposuction is a procedure which helps improve the body shape by removing unwanted and excess fat from specific areas such as the chin, abdomen, hips, thighs etc.
Liposuction is not a substitute for either exercise or an improvement in diet, though it will assist in removing any obstinate areas of fat which don’t respond to the more conventional weight loss methods.
To meet the criteria as a candidate for liposuction, you should have realistic expectations about what liposuction can do for you. Unfortunately many people assume that having liposuction will improve their whole life, whereas in fact the excess fat you are carrying may only be part of any problems you may have.
You should appreciate that although liposuction has the ability to improve your appearance and thus your self confidence, it will not necessarily alter either your looks to that of your ideal or modify the way other people treat you.
Prior to making your decision about surgery, you should think very carefully about what you are expecting and discuss these with your surgeon. If you do have any unrealistic expectations, he will be able to explain to you the realities of what is involved.
The ideal candidates for liposuction are those with a standard weight who have nice firm, elastic skin and areas of inflexible surplus fat in precise areas of their body.
It is essential that anyone considering this procedure is physically healthy secure mentally and also they have realistic expectations of the outcome of their surgery.
Although age is not a specific consideration, older patients tend to have inferior skin elasticity and they don’t usually obtain the same results as those patients with younger skin that is also tighter.
There is also a greater risk for anyone who is considering liposuction if they have any specific medical problems. These include problems such as diabetes, poor blood circulation heart disease or any recent surgery near the area to be operated on.
During your preliminary discussion with your surgeon, he will assess your health, determine where the fat actually lies in your body and weigh up the condition of your skin.
The body contouring methods your surgeon uses will be explained to you by your surgeon and it will be decided which will be the most appropriate for you. This will determine whether liposuction is the most appropriate procedure for you or whether another procedure just as a tummy tuck will be more effective.
It is important to be honest with your surgeon so that he will be able to provide you with realistic and honest answers to any questions you may have. Sometimes people considering liposuction feel bewildered about the number of options and techniques that are available to them. Your surgeon will explain each procedure and he will help you decide which the right method is for you.
It is important to research your surgeon’s qualifications along with his experience. If you consider a surgeon certified via your country’s Plastic Surgery Association., then you should be in fairly safe hands.

Who is Not a Good Candidate for Weight Loss Surgery

When diet and exercise fails to help a person lose the extra weight, weight loss surgery is often considered. This is not a quick fix and it is not an easy weight loss plan. Weight loss surgery takes time to heal, it can be expensive and it requires a complete change in diet, exercise and attitudes. There are many people who are good candidates for this type of surgery; however, there are just as many people who should not consider weight loss surgery.

Weight loss surgery requires that a person make a complete change in their attitudes and behaviors. Having the surgery alone will not result in drastic weight loss. A person who has had the surgery must learn how to eat and what foods to eat. In addition, exercise is critical after having this surgery. Doctors that perform these types of surgery generally ask their patients to exercise at least thirty minutes a day five days a week. Those having this surgery must also learn to prepare their healthy foods and learn listen to their body when they are full. People who cannot commit to these drastic lifestyle changes or do wish to change their habits are not considered good weight loss surgery candidates. For that reason, doctors ask that their patients make every possible effort to lose weight using other methods before considering this surgery.

In addition, patients who are not considered morbidly obese are not good candidates for weight loss surgery. General guidelines states that patients should be at least 100 pounds overweight or rate a 40 or above on the body mass index rating before being considered for this type of surgery. Sometimes, those who weigh 80 pounds or more will be considered if they suffer from obesity related illnesses.

Weight loss surgery is not right for people who suffer from certain diseases and illnesses. Those with certain inflammatory diseases such as gastrointestinal tract disorders, ulcers, severe esophagitis or Crohn’s disease will not be considered for this surgery. If a person has severe problems that cause bleeding in the stomach such as dilated veins or congenital abnormalities in the stomach, intestines, or esophagus or have chronic pancreatitis should also try other weight loss methods other than weight loss surgery. These conditions can cause serious problems following weight loss surgery. If you have one of these conditions, you should always let you doctor know so he or she can adequately assess your condition and determine a course of treatment.

A person who ahs experienced an injury at the surgery site, such as a gastric perforation cannot have weight loss surgery. Also those with cirrhosis of the liver and portal hypertension are not good candidate. Women who are pregnant or are planning to become pregnant should not consider weight loss surgery as an option for losing weight. This can be dangerous because there are less calories and nutrients absorbed by the body following surgery. Those who have the Lap-Band procedure can consider pregnancy after one year following their weight loss surgery.

Adjustable Gastric Banding

For those that are considering weight loss surgery, they may have found that there are several types of surgery that are available. Each of these options will differ by physician’s recommendations as well as your preference. If you are looking for the right type of surgery to help with your weight loss, then you may want to look into adjustable gastric banding. This particular type of surgery follows specific procedures that allow for you to loose the weight that you need.

An adjustable gastric band weight loss surgery begins with dividing the stomach into a lower and upper portion. The band is then placed around the upper part to make a stomach pouch. The band that is used will have an open area between the upper and lower stomach which then regulates how much food moves into one place. This will allow for you to receive the nutrients that you need and will also help with controlling your food portions. From here, the band is attached to a tube that moves to the band from your chest. This is placed in the muscles of your chest. This will be monitored in order to add or take away fluids that are in the band, causing the band to tighten or loosen.

There are several things to look for after you have had adjustable gastric banding as your procedure. For example, if the band is too tight or too loose, it will cause side effects like reflux, problems with swallowing or heartburn. However, by re-adjusting the band after these problems occur, it will prevent future problems like this from occurring. One of the reasons why the adjustable band is used is to prevent the majority of the problems and side-effects to happen.

The band that is used will continue to be adjusted to the needs that you have at the time. This will be determined by how much weight is being lost, as well as how much time has passed from the surgery. Often times, surgeons will recommend to make the band tighter or looser in order to speed up the weight loss procedure.

Tightening or loosening the band after you have had surgery will also be looked into and determined by several other signs. For example, if it is seen that you are struggling to loose one to two pounds a week, the surgeon may recommend loosening the band. If the band is too tight, you will have trouble eating and drinking fluids and will have the side-effects mentioned before. These are important to pay attention to after you have had the surgery in order to ensure your weight loss.

If you are looking into weight loss surgeries, having an adjustable gastric banding procedure done is often popular. Because it reduces side-effects, and can eliminate problems with adjustment, several surgeons will recommend this as an option for their patients. This surgery can then be a beginning to loosing the weight that you need and moving from being morbidly obese to being in complete health.

Abdominoplasty – The Tummy Tuck

Our abdomens or tummies come in many different shapes and sizes. Many environmental factors affect them such as time, gravity, pregnancies and these all take their toll on how they look. The influences of television and magazines set what we perceive as ‘normal.’ Which surgeon you choose is very important as not all surgeons will offer the same method of tummy tuck.
Our bodies are three dimensional. Excess sagging tissue often extend from the front to the sides and to the back of the body. Surgery can treat these and other arrears from ‘sculpturing’ and leaving certain parts alone.
Tummy tucks can provide very rewarding benefits as long as the patient understands the risk of complications. It is essential that a patient knows the risks involves with all surgical procedures and an Abdominoplasty surgeon should be willing to discuss and review the risks with you. The surgeon should perform a thorough medical examination of his patient before the tummy tuck surgery to identify whether that patient poses a higher risk for complications.
A tummy tuck operation is performed following the administration of anaesthesia. Anaesthesia in itself carries risks and following the tummy tuck surgery, there is also a risk of infection and blood clots. Blood clots are usually avoided by making sure the patient moves around as soon as possible following the tummy tuck surgery. Conditions such as poor healing might require another operation and it is advisable to address this with the tummy tuck surgeon before the surgery commences.
The type of tummy tuck surgery a patient will receive depends upon the amount of fat deposits they have in their abdominal area. This will also determine which will be the most effective.

During your initial consultation you will find the benefits of the tummy tuck. The surgeon should carry out a full evaluation of the patient’s health and lifestyle to decide whether a tummy tuck will be the best choice. A patient should be prepared to disclose all their medical history and their expectations. In return the surgeon will be able to explain whether a patient’s expectations are feasible. This is very important as the more information a patient provides their surgeon, the happier they are likely to be with the results. Another reason for the importance of honestly is due to the risks that can influence the outcome of a tummy tuck operation and the increased risk of complications.
During this consultation the surgeon will discuss the finances of the operation along with other aspects of the surgical procedure. The patient will be given instructions based upon their individual conditions to help prepare them for their tummy tuck. To prepare for the tummy tuck, a patient will be given guidelines regarding their diet along with other lifestyle changes that are needed. These instructions are only temporary although it is essential to follow them as they help avoid the risk of complications during and after the procedure.

Who is a Good Candidate for Weight Loss Surgery?

There is more to being a good candidate for weight loss surgery than being overweight. There are guidelines set to determine the degrees of obesity and a good surgical candidate must meet these guidelines. Candidates for weight loss surgery also must not be under the age of 18 and should be in healthy condition. These guidelines are adhered to so that patients experience optimal results following their surgery.

Weight loss surgery is a good way for those who are obese to shed the extra weight. These types of surgeries give patients gradual results, often taking at least a year to see the difference. For many, weight lose surgeries are a last resort when all other diet and exercise methods have been tried and failed. It is important for health care professionals to stick to the guidelines for weight loss surgery patients.

Patients that qualify for weight loss surgery should rate as morbidly obese on the mass body index. This index takes a person’s weight and height into consideration and finds what the person’s ideal body what is before surgery. Those who measure 40 or more on the mass body index are generally good candidates for the surgery. Sometimes, doctors will perform the surgery on these who are considered severely obese, or rates 35 to 39.9 on the index, if the person is exhibiting obesity related illnesses.

Good candidates for the weight loss surgery must also be over the age of 18. Those who are younger are not ready to have a drastic surgery for weight loss. Children’s bodies are still changing and developing. Obese children should try other methods of weight loss.

Another characteristic of a good weight loss surgery candidate is a person who has been obese for at least five years and has tried other forms of weight loss without success. Those who have been overweight for less than five years may be experiencing temporary weight gain due to lifestyle changes, stress or depression. Patients should also have tried other methods of weight loss without success. Doctors want to make certain that people are not turning to weight loss without trying to control their weight through diet changes and daily exercise. Weight loss surgery requires a complete lifestyle change, so a potential surgery patient should be ready to make this kind of commitment. In addition, weight loss surgery patients should not drink alcohol on a regular basis. This can hamper weight loss attempts and lead to problems with the digestive system following surgery.

Obesity can cause health problems such as hypertension type 2 diabetes, strokes, sleep apnea, and joint problems over a long period of time. A good weight loss surgery patient should be relatively healthy and not suffer from life-threatening diseases or illnesses. This is to ensure that they get the best medical care as possible. Lastly, weight loss surgery patients should not be pregnant and should not be planning on getting pregnant. With some weight loss surgeries, it is not recommended to become pregnant at all after the surgery. With the Lap-Band, a patient should wait 12 to 18 months after the surgery before considering pregnancy.

Plastic Surgery After Weight Loss Surgery

Those who have had weight loss surgery will find a significant difference in their energy levels as well as how they look. However, there are often times where the drastic change in weight leaves behind extra skin that was used to support the extra weight. If this is the case, then having plastic surgery can help you in going back to a normal body shape after the weight has been lost.

Weight loss surgery can allow you to loose an average of two pounds every week. This change can move you from belonging to the category of morbid obesity to being on target weight and being healthy. However the extra weight will often times stretch the skin on your body. Because of this, you may have found that you have lost the weight, but not the extra skin that was used for your body. The problem with this extra skin is that it won’t disappear with diet and exercise. This can then cause problems with looks, finding the right type of clothing and hygiene problems.

There are several types of plastic surgery procedures that you can consider having. The first is known as abdominoplasty, or a tummy tuck. This will not only remove the extra skin from this area, but will also tighten muscles that are near your abdomen. This is done by an incision being made in the abdomen. The extra skin will be removed from the incision, and then stitched back together. This is one of the more popular surgeries as most of the skin is found to stay around this area.

If the extra skin is in other areas, you can also use plastic surgery. Arms and thighs are common places to have extra skin that may need to be removed. You can also consider a face lift to remove the extra skin. Often times, not having this surgery will cause you to look older than you really are. Plastic surgery for your face can be divided to having an eye, brow or complete face lift. There are also options for having a complete lower body lift if you have several places with excess skin.

If you are considering plastic surgery to remove excess skin from your weight loss surgery and new figure, it is best to give your body some time to not only loose the weight but be ready for the surgery. The surgeries will cause your body to be stressed from the incisions and performance. Because of this, it is best to give some time before deciding to perform the surgery.

Plastic surgery is one possible step that may be implemented after you have received weight loss surgery. If you have found that you have lost a large amount of weight but the rest of your body isn’t making the right changes, then plastic surgery can help. Before looking into this, make sure that you are healthy and at your target weight. From here, you can decide the areas to get the excess skin removed; giving you a cleaner and healthier look.

Liposuction - Techniques

There are different liposuction techniques available. The basic technique involves small incisions being made on the affected area and a narrow tube or cannula, which is attached to a vacuum that sucks the fat that lies deep beneath the skin. This basic technique is used in all patients although there are several variations available.

Fluid Injection
A technique known as Fluid Injection involves a medicated solution being injected into the fatty areas prior to the fat being removed is a popular method currently being used. The fluid is a combination of intravenous salt solution and a local anaesthetic along with a drug that contract’s the body’s blood vessels. This helps the fat detach easier, reduces the blood loss and provides the patient with anaesthesia during the after the procedure. The fluid is injected to help reduce the amount of bruising following the surgery.

Tumescent Technique
The ‘Tumescent Technique’ involved the surgeon injecting large volumes of fluid, up to three times the amount of far removed. The tumescent liposuction technique is usually performed on those patients who require a local anaesthetic, although the procedure usually takes longer (sometimes 4-5 hours) than the traditional liposuction. The fluid contains enough anaesthetic so additional anaesthesia is not always needed.

Super-wet Technique
Another technique similar to the tumescent technique is the ‘Super-wet Technique.’ The difference is – lesser amounts of fluids are used. This techniques involves the amount of fluid is equal to the amount of fat that is to be removed. IV sedation is often required for this technique and sometimes a patient may need a general anaesthetic. This surgical method usually takes one to two hours.

Ultrasound Assisted Lipoplasty
Ultrasound Assisted Lipoplasty (UAL) is another liposuction technique where a special cannula that creates ultrasonic energy is used. As is passes through the fatty areas, the energy explodes to fat cell walls and the fat is liquidised. The fat is then removed using the traditional liposuction technique.
This technique can improve the effectiveness of liposuction on fibrous areas of the body, for example, the upper back or the male breast. The ease of the process is also improved. This method is often used in secondary procedure when an enhances precision is requires. This method generally takes longer to perform than the traditional liposuction method.

There will be a noticeable different in the body’s shape soon after the completion of the surgery. It must also be remembered that improvement will be even more noticeable after four to six weeks, when the swelling has settled down. Any persistent or mild swelling has usually settled down after three months and the contour will become visible. Providing that your expectations were realistic, you should be satisfied with the results of the liposuction surgery and will have more confidence when wearing a wider variety of clothes. You should be able to maintain your new shape by eating a healthy diet and taking regular exercise.

Liposuction the procedure

Before going for your surgery, you will receive specific instructions about how to prepare for your liposuction surgery. This will include specific instructions on eating and drinking, taking or avoiding certain medications and vitamins and smoking. Your surgery may also be postponed should you develop a cold or an infection. Some surgeons recommend you have blood drawn prior to your surgery in case there is a need for it during or after the surgery.
You should also ensure there will be a relative or friend available to take you home following the procedure and be available to look after you a day or two after you have been discharged from the clinic or hospital.
Liposuction is often performed in the surgeon’s outpatient centre or in a hospital. Procedures involving smaller volume liposuction are often performed in an outpatient centre, this helps minimise cost and is more convenient for may people. Should a large volume of fat be removed, or if the liposuction is being performed alongside another procedure, a night’s stay in hospital is usually required.
There are various types of anaesthesia that may be used for liposuction procedures. You should discuss these with your surgeon to decide which will provide the safest and most effective comfort during your surgery.
Liposuction may be performed under local anaesthesia if only a small amount of fat and a smaller amount of body sites are to receive liposuction. This will numb the areas affected. Some patients prefer to use local anaesthesia in conjunction with sedation to allow them to feel more relaxed whilst the procedure is taking place. An epidural block is a popular choice for more extensive procedures.
Many patients prefer a general anaesthesia, especially those who are having a large amount of fat removed. An Anaesthetist will be present throughout the procedure to ensure you are asleep during the procedure and there are no complications relating to the anaesthetic.
The duration of the procedure depends upon the size of the area to receive liposuction, the amount of fat that is to be removed and the type of anaesthesia that is to be used.
To enhance the outcome of the liposuction procedure, there are several techniques used to make the procedure easier whilst enhancing the outcome.
The procedure itself involves ensuring localised deposits of fat are removed and one or more areas of the body are re-contoured. A narrow cannula or tube is inserted through an incision in the skin and used to vacuum the fat layer sitting quite deeply beneath the skin.
The tube or cannula is then pushed and pulled through the layer of fat to break up the fat cells and suck them out. A vacuum pump or large syringe provides the suction action. If you are having many sites treated, the surgeon will repeat this procedure on each site. The incisions are kept as inconspicuous as possible.
Fluid is also lost during this vacuum process and it is essential it is replaced during the procedure to prevent complications such as shock. Patients are therefore monitored carefully and given intravenous fluids during and after the surgery.

Factors to Consider in Weight Loss Surgery

Although Weight Loss Surgery can be a life saving procedure for some people, for others it is seen primarily as a cosmetic procedure.
That said, however, it does have its advantages.
Primarily, the main benefit of weight loss surgery is weight reduction with many of its patients being pleased with the amount of weight they have lost.

That said, whenever anyone is considering undertaking weight loss surgery it is important that they discuss this fully with their doctor and seek out the best medical advice to date.
Weight Loss Surgery is not a simple procedure that requires one operation. It is a process that can take years and it is rather a drastic operation to perform – it is major abdominal surgery and assuch carries a high degree of risk.

First and foremost, it is vital that you have a clear idea of what your expectations are from the surgery and whether the results of that actual surgical procedure will be able to meet those expectations.

Along with a significant loss in weight (and often to a much more healthy and acceptable level, will come other benefits such as a lower risk of heart disease, lower
risk of diabetes, lower blood pressure as well as having more self-confidence and self-esteem as a result of being more comfortable with yourself and actually
liking what you see in the mirror (many obese people suffer from a terrible lack of self esteem as a direct result of hating their size and body).

When patients consider going for weight loss surgery, they will receive counseling to prepare them for what the surgical procedure will entail and what to expect once surgery is complete. Also, patients are advised not to expect the weight loss surgery to be
their answer to all their problems and that they will need to maintain their health and figures into the future by following proper healthy eating and exercise
plans.

As with most things in life, there are also disadvantages to bear in mind when considering undergoing weight loss surgery. Complications can and do set in, sometimes
during the surgery itself or afterwards. These can be to do with internal bleeding, DVT (deep vein thrombosis), ulcers, pulmonary problem and infection, to name
but a few.

Your doctor will also be aware of your own personal medical history and other medical problems (if you have any) so it is advisable that you seek his advice and discuss at length your thoughts prior to proceeding with the surgery.
Your doctor will be able to evaluate your medical history and present state of health and be best placed to give you advice as to whether or not you should proceed.
Not only that, but he will also be able to gauge your expectations with regard to the surgery and the reasons why you want to go this route in the first place.
